Between 10/06 19:00 and 11/06 13:50 (GMT+1:00) a subset of customers may have seen requests failing while using the dashboard at app.pusher.com. This resulted in a few users signing up for accounts and receiving welcome emails when in fact their accounts weren't fully provisioned and therefore failed. This has since been resolved and all requests should now be going through as normal.
